To lease your VeinViewer, download the Lease Application from the "Documentation" menu. Complete the form by entering your information into the editable fields (noted by dark grey highlights), print, sign and fax the Application at your convenience.

Note: A signature is required for Luminetx to process the Lease Application. Luminetx will process your completed form and will contact you upon review of your Lease Application.
